      Members requesting credit from an Energy Cooperative Supplier understand that a credit check may be performed before any service will be provided.  I/We understand that this application is for heating oil only; a separate contract is required for electricity.  I/We proclaim that the information provided on this application is correct and true. 
       Account balances are due 10—15 days from invoice date, depending upon Supplier, and this form of payment may require a credit card on file with the supplier. Delinquent balances may be assessed finance charges according to Supplier’s standard procedure.  Suppliers reserve the right to discontinue service and pursue collection actions on accounts with delinquent balances.
